I acknowledge the inherent risk of exposure to COVID-19 or other diseases that exist in any public place where people are present. By submitting this form, I certify that my child is in good health and able to participate safely in any activity in which I have registered them to participate in, including all functions and events in association with Dancers Only Studio. In considerations of Dancers Only Studio allowing the above named student to participate in its dance/gymnastic classes, camps, drop-ins, performances, and rehearsals, I do hereby forever release and hold harmless Dancers Only Studio, its owner, employees, volunteers, teachers and/or assistants, and staff from any and all claims of every kind of nature, whether in negligence or otherwise, for any illness, arising out of the student's participation in the above mentioned activities or any activity associated with Dancers Only Studio
